Subfascial Breast Augmentation
Subfascial implant placement is one of 4 possible options, though most think of breast implants as simply going over the muscle or under. (The fourth option is the split muscle plane.) Subfascial (pronounced like fashion, not facial), is a versatile alternative that balances often competing factors between going over or under the muscle. I published the first article on the procedure* in North America in 2005 and have been using variations of it, routinely. Subfascial strikes a balance between coverage and implant support and is especially well suited for athletic women.
The fascia is a thin, but reasonably tough, layer on the surface of muscles that consists of connective tissue. By carefully lifting the fascia off of the pectoral muscle, a space for the implant can be created that leaves the muscle intact. While not adding padding the way the muscle does, the fascia is often enough to smooth the transitions around the implants and add support.
Split Muscle Breast Augmentation
Most breast implants are placed under the pectoral muscle because they usually appear more natural. However, what many don’t realize is that the usual method – called “dual plane” – involves cutting the muscle from its attachment on the chest in order to allow the implant to rest in the proper position. This part of the muscle heals onto the scar capsule in front of the implant, where it can cause animation deformities (reported to occur in more than 3 out of 4 patients) and double bubble. I developed the split muscle technique many years ago in order to prevent and correct these problems. Basically, it preserves muscle coverage over the upper part of the implant (where it is most needed), maintains the muscle attachments so there is no loss of strength or function, and significantly minimizes animation problems and double bubble. I have used it on more than 1,000 patients for over 10 years.
